Does my signature have to be the same as my legal name on clinical research regulatory documents?

I've wondered that too. I have always gone by my nickname, not my legal name. When I sign documents I include my nickname in parentheses in front of my full legal name so people will know I was the one who signed off. Is that correct method?

I have the same question too, my official signature is signing just my first name, which is my whole and complete signature. I have had people insisting on including my last name in my signature. Is there any binding to do so?
